Keep in mind that the questions on this survey are very basic, and not comprehensive in any fashion. I'd love to know more detail about your campaign than these questions cover, but I want to start things off simply.

Your answers will help guide me if I want to follow up with you more, and the more detail you can share now about these questions, the more likely I'll end up including some of your stories in the book.
